NEW PASSPORT GENERAL REQUIREMENTS

• Personal appearance

• Confirmed appointment

• Application Form (Duly Accomplished)

• PSA issued Birth Certificate 

• If Married, PSA issued Marriage Contract

• Two (2) valid Government picture IDs and at least two (2) supporting documents, both with photocopies.

PASSPORT RENEWAL GENERAL REQUIREMENTS

• Personal appearance

• Confirmed appointment

• Application Form (Duly Accomplished)

• Recent Expiring or Expired Passport

• Photocopy of your Passport is DATA PAGE

• Additional Requirements:

  • At least one (1) valid Government picture IDs and at least two (2) supporting documents, both with photocopies. 

  • PSA issued birth Certificate

  • For change of status, PSA issued Marriage Certificate.

 

MINORS:

• Personal appearance of the Minor applicant

• Personal appearance of either parent and valid passport of parents

• Marriage Certificate of Minor’s Parents issued by PSA

• Original Birth Certificate of Minor issued by PSA (Other option for Infants : Certified True Copy of Birth Certificate issued by the Local Civil Registrar and duly authenticated by PSA)

 

LOST PASSPORT GENERAL REQUIREMENTS

• Personal appearance

• Confirmed appointment

• Valid picture IDs and supporting documents to prove identity. 

• PSA issued Birth Certificate 

• If married, PSA issued Marriage Certificate 

·Submit Notarized Affidavit of Loss (with detailed explanation on when, where and how passport got lost).

·If the lost passport is still valid, submit Police Report.

·Photocopy of first page of lost passport (if available)

NOTE:  There will be a 15-day clearing period prior to the processing of application for replacement of a lost valid passport.
